Juvo Insurance Solutions Inc. is an independent agency and is NOT affiliated with, endorsed by, or under contract with CMS, Medicare, Medicaid, the Social Security Administration, or any government agency.

• Government Identifiers: Medicare ID and Medicaid ID (where required for enrollment)
• Health Information (PHI): Insurance plan information, medical providers, medications, and eligibility status

Juvo communicates with clients via two business phone numbers — one for direct agent communications and one for automated marketing and appointment messaging. Pursuant to CMS regulations: Personal beneficiary data will only be shared with another TPMO when prior express written consent is obtained from the beneficiary.
