As a licensed insurance agency that handles Protected Health Information (PHI) in connection with Medicare and other health insurance products, we are required by HIPAA to maintain the privacy of your PHI and to provide you with this Notice.

  • LIS/Extra Help and Medicare Savings Program eligibility

  • Healthcare Operations: For internal compliance, quality assurance, and staff training
  • Required by Law: When required by federal or state law or government authority
  • CMS & Carrier Reporting: As required by CMS TPMO regulations and carrier requirements
  • Call Recording: All marketing, sales, and enrollment calls are recorded as required by CMS TPMO regulations
4. Uses Requiring Your Authorization

For uses and disclosures not described in this Notice, we will obtain your written authorization. You may revoke any authorization in writing at any time.

5. Your Rights Regarding Your PHI
  • Right to Access: Request access to and copies of your PHI
  • Right to Amend: Request correction of PHI you believe is incorrect
  • Right to Request Restrictions: Request restrictions on how we use your PHI
  • Right to Confidential Communications: Request communications in a specific way
  • Right to a Paper Copy: Request a paper copy of this Notice at any time
  • Right to Breach Notification: You will be notified of any breach of your unsecured PHI
6. Our Duties
  • Maintain the privacy of your PHI
  • Provide you with this Notice of Privacy Practices
  • Follow the terms of the Notice currently in effect
  • Notify you in the event of a breach of your unsecured PHI
7. Security Safeguards

We protect your PHI through encrypted communications, access controls, multi-factor authentication, data protection agreements with all technology vendors, and restricted access limited to authorized Juvo personnel only.

8. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us through our website contact form or by calling (626) 251-9884. You may also file a complaint with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.

9. Changes to This Notice

We reserve the right to change this Notice and make new provisions effective for all PHI we maintain. The current version will be available on our website.
